Adding, editing, and discontinuing a client's medications

How managers add a scheduled or PRN medication on the Client 360 Medications tab, edit the details, and discontinue a med while keeping it for audit.

A client's medication list lives on their profile, under the Medications tab. That list is what feeds the Med Pass grid your DSPs work from, so getting it right here is what makes med passes accurate everywhere else.

Managing medications is for SUPER_ADMIN, ADMIN, DIRECTOR, and MANAGER roles. DSPs can record doses but can't add, edit, or discontinue a med. If you don't see the Medications tab at all, the module may still be off — an admin can turn on eMAR first.

Add a medication

  1. Open the client's Client 360 profile and go to the Medications tab.
  2. Under Active medications, click Add medication.
  3. Fill in Name * (required). Add Strength, Prescriber, Form, Route, Pharmacy, Rx #, and Instructions (sig, optional) as you have them.
  4. Set a Start date *, and an End date (optional) if the med is meant to stop on a known day.
  5. Click Add medication to save.

Scheduled or PRN

Every med is either given on a schedule or given only as needed. The PRN (as needed) toggle decides which.

Scheduled (toggle off) — the med appears on the Med Pass grid at set times each day.

  1. Add one or more Times * rows with Add time. A scheduled med needs at least one time.
  2. Optionally set Days of week. Leave it empty and the med is due every day.

PRN (toggle on) — "Given only when indicated, not on a fixed schedule."

  1. Turn on PRN (as needed).
  2. Enter a PRN indication * — the reason it's given, like "for pain" or "for anxiety." This is required and shows to staff when they record it.
Times are Eastern wall-clock. A dose set for 8:00 AM means 8:00 Eastern, every scheduled day.

The controlled-substance badge

Turn on Controlled substance to mark a med. This adds a display-only badge so staff can see it at a glance. It does not add pill counts, witness fields, or any special workflow — it's a label, nothing more.

Edit a medication

Reopen a med from the Active medications list to change its details — a new strength, an added time, a corrected prescriber. Edits update what staff see on the next med pass. Anything already recorded stays as it was recorded; editing the med doesn't rewrite past doses on the MAR.

Discontinue a medication

When a med stops for good, discontinue it — don't delete it.

  1. On the Medications tab, find the med and click Discontinue.
  2. Enter a Reason *. This is required.

The med moves to Discontinued history, which keeps the Name/Strength, the date it was Discontinued, who did it (By), and the Reason. It stops appearing on new med passes and shows as "(DC)" on the MAR. Nothing is ever hard-deleted from the UI — discontinuing is permanent but fully preserved for audit.

When the list is set, your DSPs are ready to pass meds on the Med Pass grid.
